Description: Basic concepts of patient navigation including disease prevention and community health, health insurance navigation, as well as accessing and analyzing health information. Training in assisting individuals with limited health literacy to access the maximum array of benefits from community services, clinical care, and health insurance.

1. Define the role and purpose of the patient care navigator position. (I)
2. Explain the need for patient advocacy and care coordination. (I)
3. Identify the causes of barriers to care and how care coordination and patient navigation can reduce health care disparities. (II)
4. Discuss current health care legislation, policy, and ethics as they apply to health care and the patient navigation role. (III)
5. Describe the basics of health insurance and paying for health care services. (III)
6. Describe how effective communication and cultural awareness affect patient outcomes. (IV)
7. Identify skills needed for accessing, analyzing, and effectively communicating health information. (V)
